TRT in the Philippines

Types of Testosterone Available in the Philippines

Testosterone enanthate, undecanoate (Nebido) and gel compared: how each works, peaks and troughs, Philippine availability and the cypionate question.

All testosterone used in TRT is the same hormone. What changes between products is the ester (a chemical tail attached to the molecule that controls how quickly it is released) or the delivery route (into a muscle or onto the skin). Those differences decide how often you need treatment, how steady your levels are, and what it costs.

In the Philippines, three forms are registered and sold on prescription: testosterone enanthate, testosterone undecanoate and testosterone gel. Two others that are common abroad, cypionate and Sustanon, have not been confirmed on the FDA Philippines verification portal. Below is what each one is, how they compare, and what to ask your doctor.

This page is disease education. Generic names come first and brands in parentheses. We do not rank or recommend brands, and we do not give doses or schedules, which are your doctor's call.

Testosterone enanthate

Testosterone enanthate is the workhorse of TRT in the Philippines. It is a generic, registered, shorter-acting injectable that is widely used around the world and has been for decades. It is also by far the lowest-cost option here. DIMA Health, a licensed online pharmacy, lists it per ampule at about ₱167.

How it behaves in the body

After an injection, the enanthate ester slowly lets testosterone into the blood. Levels climb after the injection, peak, then fall gradually until the next one. Because it is shorter-acting, it is given more often than long-acting forms.

That rise and fall is what people mean by peaks and troughs:

  • Near the peak, some men feel energetic and have a higher sex drive. A high peak can also bring oily skin, acne or irritability, and pushes red blood cell production up.
  • Near the trough, levels drift low, and symptoms like tiredness or low mood can creep back before the next injection.

Doctors manage this by adjusting the dose and timing for each person, and by checking levels at a consistent point. That fine-tuning is exactly why enanthate should be supervised, not self-managed.

Who it tends to suit

Men who live or work near their clinic, who are fine with injections, and who want the lowest medicine cost. It is also the form most often used at men's health clinics in Makati, BGC and Pasig. For a cost breakdown including the extra clinic visits, see how much TRT costs in the Philippines.

Testosterone undecanoate (Nebido)

Testosterone undecanoate is a long-acting injection. The undecanoate ester is longer and releases far more slowly, so injections are given at clinic visits months apart rather than frequently.

How it behaves in the body

Levels rise gently after an injection, stay in range for a long stretch, and taper slowly. There are no sharp peaks, so men who felt the ups and downs of shorter-acting injections often find it steadier.

The injection is larger in volume and is always given by a clinician at the clinic. A rare reaction involving a cough or breathlessness right after the injection has been described, which is why the clinic watches you briefly afterward.

Cost

Per injection, retail pharmacies list ₱12,543, which spread over the months between injections works out to about ₱4,000 to ₱5,000. It is the most expensive medicine line, but the fewest clinic visits.

Who it tends to suit

Men who dislike frequent injections, live far from a clinic, travel a lot for work, or found shorter-acting injections uneven. It is also popular with OFWs and seafarers who need treatment that fits around long periods away, although that needs careful planning with a doctor.

Testosterone gel (AndroGel and others)

Testosterone gel is applied to the skin and absorbed into the blood. It is applied at home as part of a regular routine.

How it behaves in the body

Gel gives the steadiest levels of any form, closest to how the body makes testosterone naturally, with no peaks and troughs worth speaking of. That makes it a good fit for men sensitive to swings. The large TRAVERSE heart-safety trial in 2023 used testosterone gel.

Practical points

  • Transfer risk. Testosterone can pass to a partner or child through skin contact before the gel dries or if the area is not covered. Doctors advise washing hands and covering the area. This matters a lot in Filipino homes where small children are often carried and cuddled.
  • Heat and sweat. In Manila heat, a commute or a jeepney ride soon after applying can affect how well it absorbs. Ask your doctor or pharmacist about timing.
  • Absorption varies. Some men absorb gel poorly and need their level checked and their prescription adjusted.

Cost

Per 88 g pump, the range is ₱2,459 to ₱2,589. It costs more per month than generic enanthate but removes needles and injection visits.

Testosterone cypionate: registration unverified

Testosterone cypionate is the most common TRT injection in the United States, so many expats arrive on it and many Filipino readers see it in American videos and forums. Chemically, it is very similar to enanthate: a shorter-acting ester with a slightly slower release. Most doctors treat the two as close equivalents.

Its registration is not yet confirmed on the FDA Philippines verification portal. We therefore do not say it is available in Philippine pharmacies and do not list a price. If you see cypionate offered locally, check the product's registration number on the portal before trusting it. If you were on cypionate abroad, our guide to continuing your prescription after moving to the Philippines explains how doctors handle a switch to a registered local form.

Testosterone esters blend (Sustanon): registration unverified

Sustanon is a brand of injection that blends several testosterone esters, some fast and some slow, in one shot. The idea is a quick early rise and a longer tail. In practice, levels can still peak and dip, and many doctors prefer single-ester products for more predictable results.

Sustanon's registration is not yet confirmed on the FDA Philippines verification portal, so we do not describe it as available here. The name is common in gym circles, and fake versions are a known problem worldwide.

What about pills and "online TRT"?

You may see testosterone tablets or capsules advertised on social media, online marketplace listings or gym group chats. Products sold this way are usually not testosterone replacement at all. Many are unregistered oral anabolic steroids, which FDA Philippines has warned against, and some are counterfeit. Oral steroids can be hard on the liver and none of them come with a doctor checking your blood.

TRT in the Philippines is prescribed by a doctor and dispensed by an FDA-licensed pharmacy. We explain the difference between TRT and steroid use in TRT vs steroids.

How doctors choose a form with you

There is no single best form. Doctors usually weigh:

  • Budget: generic enanthate costs the least; undecanoate and gel cost more.
  • Time and travel: long commutes, shift work or frequent travel favour fewer clinic visits.
  • How you feel on swings: men sensitive to peaks and troughs may do better on undecanoate or gel.
  • Household: gel needs care around children and partners.
  • Hematocrit: forms with higher peaks tend to push red blood cells up more, which affects monitoring. See TRT side effects and risks.
  • Fertility plans: all forms of testosterone suppress sperm production, so this needs to come up before choosing any of them.

Whichever form you start on, your doctor will check your testosterone level and safety markers a few months in, and can switch forms if one does not suit you. For the whole process from diagnosis to follow-up, read our complete TRT guide.

Frequently asked questions

Frequently asked questions

Is testosterone enanthate available in the Philippines?

Yes. Generic testosterone enanthate is registered and dispensed by Philippine pharmacies on prescription. It is a shorter-acting injection, usually given at the clinic.

Is testosterone cypionate available in the Philippines?

We have not been able to confirm cypionate's registration on the FDA Philippines verification portal, so we do not say it is available in Philippine pharmacies. Men on cypionate abroad usually discuss a registered local form with their new doctor.

What is the difference between enanthate and cypionate?

They are very similar shorter-acting injectable esters of the same hormone. Cypionate releases slightly more slowly, but in practice doctors treat them as close equivalents. The bigger practical difference here is registration status.

Is testosterone gel better than injections?

Neither is better for everyone. Gel gives the steadiest levels and avoids needles, but it costs more and can transfer to others by skin contact. Injections cost less and are given at the clinic, but levels rise and fall more. Your doctor helps you weigh these.

What is Nebido?

Nebido is a brand of testosterone undecanoate, a long-acting injection given at clinic visits months apart. It gives smoother levels than shorter-acting injections and means fewer visits, at a higher price per injection.

Are oral testosterone pills sold online real TRT?

Pills sold online as testosterone are usually unregistered anabolic steroids, not testosterone replacement. FDA Philippines has warned against unregistered steroid products. Check any product on the FDA verification portal.
