Cebu is the medical and commercial hub of the Visayas, and the natural base for anyone on testosterone replacement therapy (TRT) in the central Philippines. It draws Filipino professionals from across the region and a large community of foreign retirees, many of whom later settle on Negros, Bohol or Leyte. Demand for TRT here is real. Supply, especially of specific forms, is less predictable than in Metro Manila, so planning matters more.

This page covers who provides TRT in Cebu, what is and is not available, labs, and how to keep treatment going if you move to a smaller island.

Beyond these, the large private hospitals in Cebu City have endocrinologists and urologists in their outpatient departments, and that is a sound route if you want hospital-based care or have other health conditions. Telehealth services that operate nationwide also cover Cebu; see where to get TRT in the Philippines for the full list.

Visayas demand, and why planning matters

Expat forums tell a consistent story. Retirees arrive in Cebu already on treatment, find a doctor, then move to Dumaguete, Siquijor, Bohol or Leyte for a quieter life, and discover that the nearest pharmacy carrying their form is a ferry ride away. One retiree on cypionate who was choosing between Cebu, Negros, Leyte and Bohol reported that a clinic had already warned him about a shortage before he arrived.

Locals face a version of the same problem. A man in Mandaue or Lapu-Lapu can usually find what he needs; a man in a provincial town may need to travel to Cebu City for the consult and the lab work.

The answer in both cases is the same: set up care in Cebu first, agree a plan with your doctor, and build in a buffer.

The cypionate gap

Testosterone cypionate is the most common TRT injection in the United States, and many American retirees arrive on it. In the Philippines, its registration is not yet confirmed on the FDA Philippines verification portal, and patients regularly report that pharmacies do not carry it. The testosterone esters blend (Sustanon), common for Australians and Europeans, is in the same position.

What is registered and available:

  • Testosterone enanthate (generic): a shorter-acting injection given more often. It is the closest equivalent to cypionate, and the form most American patients in Cebu move to.
  • Testosterone undecanoate (Nebido): a long-acting injection given at clinic visits months apart. Useful if you live far from a pharmacy, because there are fewer refills to manage.
  • Testosterone gel (AndroGel): applied to the skin, no injections.

How to switch between forms is a decision for your doctor, who will usually recheck your labs after the change. We explain the general approach in continuing TRT after moving to the Philippines, and compare the forms in types of testosterone.

Labs in Cebu

Cebu City, Mandaue and Lapu-Lapu have plenty of laboratories: hospital labs, national diagnostic chains and independent clinics. Testosterone and the standard TRT panel (hematocrit or full blood count, PSA, estradiol, lipids) are routine tests. Results usually come back within a day or two.

For reference, a total testosterone test costs $39 at Medicard's walk-in rate. Other lab options are compared in our testosterone test price guide.

If you are being diagnosed for the first time, guidelines call for two early-morning total testosterone tests on separate days, plus symptoms, before a diagnosis of low testosterone. Book morning draws, ideally before 10 am.

Your first appointment in Cebu

Bring as much of your history as you can. For patients already on treatment, that means your original diagnostic labs, your most recent results (ideally from the last three to six months), your current prescription and a short letter from your previous doctor. A photo of your current product label helps the doctor see exactly what you were using.

Expect the doctor to take a full history, ask about other conditions such as high blood pressure, sleep apnoea and prostate problems, and order a fresh baseline panel if your results are old or missing. They will then discuss which registered form suits you, whether injections will be given at the clinic or at home, and when to repeat labs.

New patients without a diagnosis should start with the two morning tests and a consult. Treatment comes after the results, not before.

Planning around island moves

If you are moving from Cebu to Negros, Bohol, Leyte or another island, use this checklist before you go:

  1. Get established in Cebu first. A Cebu-based doctor or telehealth provider who knows your history is your anchor, wherever you end up.
  2. Ask about telehealth follow-ups. Many doctors can review labs and renew prescriptions online, so you only travel for the visits that need you there in person.
  3. Find a lab near your new home. Even small cities usually have a hospital lab or diagnostic centre that can run testosterone, a blood count and PSA from a doctor's request.
  4. Identify a licensed pharmacy that stocks your form, or one that delivers. Under FDA Advisory 2019-154, medicines can only be sold online by FDA-licensed pharmacies, and some deliver to the provinces against a valid prescription.
  5. Refill early. Ferries are cancelled during typhoons and rough seas, and courier deliveries to islands slow down. Do not let your supply run close to empty.
  6. Agree a fallback with your doctor. If your form runs out locally, know in advance what the alternative will be.
  7. Carry your paperwork. Keep your Philippine prescription and recent labs with you when you travel between islands.

If you are considering long-acting testosterone undecanoate (Nebido) because clinic visits months apart suit island life, raise it with your doctor; it changes how often you need a pharmacy, but it also changes how monitoring is timed.

At-home injecting

Many retirees on the islands would rather inject at home than travel to a clinic. Whether that is appropriate is a conversation to have with your doctor. Some are comfortable with it after training and with regular follow-up labs; others prefer to give injections at the clinic. We do not give injection instructions on this site.

Costs

Budget for consults, labs and medication. A specialist consult typically costs $25 to $75, and the medication is the cheaper part of the total. Travel from the islands to Cebu City, and any home-service lab fees, add to the bill. Our TRT cost guide has the full breakdown.

What to avoid

The shortage problem makes grey-market sellers more tempting. Facebook pages, gym contacts and websites using "TRT" in their names offer vials and oral tablets with no prescription. Those products are often unregistered or counterfeit, and nobody is monitoring your blood while you use them. You can check any product on the FDA verification portal. Our page on TRT vs steroids explains the risks.

Coming soon: more Visayas coverage

We are working on pages for Dumaguete and Negros Oriental, and for Bohol, and will publish them once we can list at least one provider with checked facts. Until then, Cebu-based doctors and nationwide telehealth services are the most reliable routes.
